The parachutiste made 3-way flat relative work jump from 4,000 meters which was uneventful, as was the brake off and deployment.
On trying to release the brakes, the right hand braked released normally but the left did not. He let go of the right hand brake to use both hand on the left. The parachute entered a tight left hand spiral and started to descend rapidly. He failed to release the left brake, failed in an attempt to stop the turn with the right brake toggle. He lost awareness of altitude, when he made his decision to cut away experienced some difficulty locating the cutaway handle, probably due to the speed of rotation. He release the main and pulled the reserve handle just after the Vigil had fired.
First jump on his new rig . 2 way jump . On the altitude of the opening , can't reach the pilot chute . Start look for it with both hands , and get in diving . Stabilized after some seconds ,and repeet , the same scenario ,and lost the stability .
Le client n'a pas trouvé sa poignée d'ouverture du principal et n'a pas actionné son parachute de secours. L'appareil à donc déclenché l'ouverture du parachute de secours.
Activated during a wingsuit jump, skydiver got ready to pull couldn't get the main handle ended up flipping over. Tried again and ended up flipping to back and then into a head down spiral. By the time skydiver was able to get control and pull the handle AAD fired and was under reserve.
After brake off , jumper could not pull pilot chute. Then started searching for reserve handle but was never pulled out of Pocket.
Wingsuiting at 4000ft i reached for my pilot chute, found it but couldn't pull it out (had several hard pulls before with new rig) Went back into stable flight position and tried to pull again until I got my 2000ft alarm. Grabbed my reserve handles and wanted to cutaway (out of habbit for emergency drills)
Released reserve handle cause I needed 2 hands for peeling off my cutaway pillow, went in stable position again for maybe 2sec or so waiting for skyhook that didn't work ofcourse :) reached again for reserve handle and pulled but found out later my Vigil just fired before my pull. Landed safely in the forest.
Jumper dislocated arm during freefall
Exit from the aircraft at an altitude of 4000 m, stable. Fell steadily. Made wave-off on 1200 m. Not found hackey handle of pilot chute. On 800 m I tried to pull reserve handle, but was looking for the reserve ring for long time and found it on 400 m and pulled it. But the Vigil was activated allready
A solo jumper decided not to open main because of unstable freefall and take a bad decision not to open the reserve by himself.
5th jump of day in wingsuit. Can't find the pilot chute of main. 5 trials i have cutaway (RSL was off) and trying to deploy reserve. But because of the oncoming flow to the wing, i can't reach a hand to the release.Vigil fired and deployed the reserve. With my immense gratitude.
