Angle jump (friend on belly and me on back). We didn't hear our dytter. My friend has moved on a side to make very low opening, during his separation, my reserve opened when i was on my back. I only understood that we were too low when i saw my reserve passed between my legs.

The first jump after a long break. Disordered fall. No action was taken. landing the reserve parachute in the wind and broke his arm.
Jumper was disoriented and din't pull. The loop was cut.
Fell into a disorderly fall, tried to stabalize but did not work. Seperating the stabilization link tried to reach the jellufish link, until the divice triggered and released the reserve.
Wingsuit jumper lost control upon attempting to open main. Coach on the load observed the instability and jumper was tumbling.
On the date of the incident, he was given a refresher cllass by an AFF Instructor. The same instructor accompanied him on this recurrency dive and initially had a harness hold on him. When he relased the jumper, the jumper started to spin, Instructor re-docked and stopped the spin and re-released. Jumper started spin again, again Instructor stopped spin and gave him appropriate hand signals to correct. On re-release, jumper started violent spin, floated up in the spin, and Instructor was unable to re-dock but gave jumper the pull signal at approximately 7,000'.
Jumper did not pull and continued in a violent spin until the AAD fired, at which appeared to be the proper altitude. Jumper landed under the reserve with no injury.
