| Apon exit with a wing suit i went on my back and started to spin. I attempted to correct the spin wich turned into a tail spin around 4000ft. At that time the spin became so violent that i became disorianted and went for my main handle. | On saturdayAugust 20th, an AAD fire on a Tandem occured. Skies were clear and winds were light. The cause of the fire was due to no drogue being thrown and no pull on the reserve handle. When the instructor was asked to describe the situation, she stated that she tought she has releases were pulled. Both the instructor and student landed without incident. The instructor believes that the cause of this incident was extreme fatigue. This instructorhad done more than 20 Tandems at this DZ, and were no prior safety concerns. |

| At 2500 ft AGL, i attempted to deploy the pilot chute. My hand slipped off. I tried again but when i did so, i dropped my left shoulder and rolled over.
