| On saturdayAugust 20th, an AAD fire on a Tandem occured. Skies were clear and winds were light. The cause of the fire was due to no drogue being thrown and no pull on the reserve handle. When the instructor was asked to describe the situation, she stated that she tought she has releases were pulled. Both the instructor and student landed without incident. The instructor believes that the cause of this incident was extreme fatigue. This instructorhad done more than 20 Tandems at this DZ, and were no prior safety concerns. |
